Dr. Nitin Putcha, President Hospitalist & SNFist Services
Dr. Nitin Putcha is a board-certified physician. In 2014, he founded National Health Rehabilitation (NHR), a physician-led practice dedicated to enhancing patient outcomes through the integration of physician services in post-acute care settings. Under his leadership as Chief Executive Officer, NHR expanded its reach and impact in the healthcare sector.
Throughout his career, Dr. Putcha has combined clinical expertise with entrepreneurial acumen to advance the field of healthcare management via provider services. His efforts have been instrumental in integrating specialized and primary care services into broader healthcare delivery models, thereby improving patient care in acute and post-acute settings.
His role with IPMGT is focused on tailoring growth strategies to healthcare companies and facilities alike to reach their short and long-term growth initiatives primarily via fractionalization of support services, and capitalization of synergies.

Dr. Michael Zaplin, Chief Medical Officer
Dr. Zaplin serves as CMO of IPMGMT. A part of his role is to manage hospital and SNF stays with alternate payment structures with VBC elements. Current clients include self-insured primary care groups (full risk and partial, as well as ACOs). Dr. Zaplin has years of experience throughout the clinical continuum to drive operational efficiencies leading to improvement in key KPIs that drive value-based care.

Dr. Amishi Dharia, EVP of Special Projects
Amishi Dharia is Executive Vice President of Special Projects for IPMGMT. Dr. Dharia is a board-certified physician. She co-founded and served dually as Chief Operations Officer and Chief Medical Officer of National Health Rehabilitation. She currently leads clinical operations, clinical integration, coding integrity, compliance processes, risk mitigation strategies, and education development.

Sarah Hatchel, SVP of Clinical Operations
Sarah Hatchel serves as Senior Vice President of Operations at IPMGMT, supporting hospitalist groups with a strategic focus on improving operational performance across acute and post-acute care settings. With a deep expertise in provider alignment, care coordination, and clinical workflow optimization, she has led multi-site teams and supported key initiatives to enhance quality and efficiency.
Sarah’s background in both rehabilitation and healthcare operations enables her to bridge clinical and administrative functions, ensuring alignment between strategy and patient care delivery. She has played a key role in integrating operational systems and advancing value-based care initiatives. Her leadership continues to support cohesive care models and improved transitions across the continuum.

David Taback, VP of Business Development
David Taback is a healthcare operations executive with extensive experience in hospitalist program leadership, physician group management, and facility relations. He works to successfully bring value-based care initiatives into the hospitalist space, enhancing physician decision-making and supporting more coordinated, patient-centered care.
David has led the expansion of hospitalist services across multiple hospitals and oversees administrative teams that support physician engagement, clinical alignment, and operational efficiency. His background includes managing patient engagement infrastructure, scaling care coordination teams, and implementing data-driven strategies within accountable care organizations.
With a strong foundation in health services and data analytics, David brings a strategic and systems-oriented approach to optimizing hospitalist operations.

Senior Director, Mergers and Acquisitions
Vikram Eagam is an experienced healthcare leader with a strong foundation in finance and operations. With over a decade of experience spanning healthcare consulting, risk advisory, and provider operations, he has led strategic initiatives across the industry. His career has included advising private equity firms on their healthcare portfolio companies, executing post-acquisition integration strategies, and managing compliance risk in M&A transactions.
As Senior Director of Mergers and Acquisitions at IPMGMT, Vikram oversees the end-to-end integration of acquired practices, ensuring a seamless transition across clinical, operational, and administrative functions. He leads efforts to align new partners with IPMGMT’s systems, monitors performance milestones, and identifies opportunities to enhance efficiency and value.

Dr. Trupti Patel, Clinical Documentation Specialist
Dr. Trupti Patel serves as Clinical Documentation Specialist at IPMGMT, where she leads initiatives to enhance quality, provider performance, and regulatory compliance. With over 17 years of progressive experience in healthcare administration, Dr. Patel brings a unique blend of clinical insight and strategic leadership to the forefront of revenue cycle optimization and operational excellence.
Her career is distinguished by a proven ability to align medical expertise with business acumen, driving transformative improvements across complex healthcare systems.
Dr. Patel excels in building collaborative, cross-functional teams and streamlining workflows to improve coding accuracy, revenue capture, and compliance outcomes. Her leadership continues to shape high-performing, compliant healthcare environments that prioritize both patient care and organizational integrity.
